How do I submit a form?
- Open the form in your default PDF reader (i.e. Adobe, Google). Fill out all required sections of the form and click "submit."
- Download (save) the form to your desktop, or other desired location on your computer. Fill out all required sections of the form, save the completed form and email the form to the appropriate email address (listed on the form).
- Print out the form. In print, complete all required sections of the form by hand, neatly and legibly.
[Note: if you choose to submit a form or request using the this method, you must complete ALL REQUIRED SECTIONS or the application will not be processed. Also, if the handwritten sections cannot be read, you will be asked to resubmit the form using an alternative method]
Having trouble finding the form you need?
- Locate the form or document that relates to your request, or needs (tip: sort or search for the form # for the most accurate results).
- Open the form or document by clicking the icon under "Open."
